Study Purpose

The primary aim of the Community Health Status Assessment (CHSA) is to pinpoint priority health and quality of life issues within the community. By addressing questions such as "How healthy are our residents?" and "What does the health status of our community look like?" this phase collects extensive data to comprehensively understand the community's health. The insights gained will form the basis for identifying strategic health issues and creating targeted improvement strategies in future phases. The MAPP process emphasizes community and stakeholder involvement to ensure that health initiatives are well-informed and driven by community needs.

Background:

Mobilizing for Action through Planning and Partnership (MAPP) is a community-driven strategic planning process designed to improve community health. Developed by the National Association of County and City Health Officials (NACCHO) in collaboration with the Centers for Disease Control and Prevention (CDC), MAPP helps communities apply strategic thinking to prioritize public health issues and identify resources for addressing them.

Project Phases

Phase 1: Organizing for Success/Partnership Development

This initial phase focused on organizing a diverse partnership of community stakeholders, laying the groundwork for a successful MAPP process. The foundation was set for a unified approach to community health improvement by fostering collaborative relationships.

Phase 2: Visioning

In the visioning phase, community members and stakeholders collaborated to create a shared vision for the future. This vision serves as a guiding star for all subsequent efforts, ensuring that all activities align with the community's health and quality of life aspirations.

Phase 3: The Community Health Status Assessment

The ongoing third phase, the Community Health Status Assessment, thoroughly evaluates the community's health. This phase is critical for understanding current health outcomes and identifying areas for improvement. By analyzing data from various sources, the assessment provides a detailed picture of the community’s health status, which is essential for informed decision-making in future phases.

Methodology:

The CHSA employs a multifaceted approach to gather data on community health. Key components include:
 

  • Quantitative Data Collection: Utilizing health surveys, public health records, hospital data, and other statistical resources to gather measurable health indicators.
     

  • Qualitative Data Collection: Conducting focus groups, interviews, and community forums to capture community members' lived experiences and perceptions regarding their health and quality of life.
     

  • Stakeholder Engagement: Involving a wide range of stakeholders, including healthcare providers, community leaders, non-profit organizations, and residents, to ensure diverse perspectives are included in the assessment.
